Most people finish their set in 20-30 seconds. I believe this is why they are not getting the results they are looking for. Instead, aim for closer to 60 seconds per set.
2.
If the goal is to get results in terms of body transformation you need to feel the intensity and know that the muscle is working. Intensity is the key and has been shown to result in more hypertrophy.
3.
You can get results in any rep range as long as the time under tension and intensity is there. Using both to your advantage will skyrocket your results!
